0114 Support enabling providers to offer community nursing care to participants in their home to respond to the disability-related health needs of a participant where that care is not the usual responsibility of the health system.

This is where participants require higher levels of care and support and may involve assistance with: Personal care, medication administration, supporting participants in the use of personal equipment including standing or walking frames, wheelchairs, hoists, sliding sheets or moving boards, hearing aids and other physical aids.
